What is CONNECTIONS?

Early intervention to memory loss.

Our CONNECTIONS program is a holistic, early intervention service that enhances abilities, supports cognitive changes, and creates opportunities for rich and meaningful connections for assisted living residents experiencing early memory loss.  

We focus on diet, exercise, sleep, enhancing the resident environment, reducing pharmacology, maintaining family connections, and being part of a larger community.  We challenge the intellect, remove barriers to engagement, and promote autonomy in a small-group, structured setting with peers who are experiencing a similar situation.

What are the early signs of memory loss?

 Mild cognitive impairment (MCI) is quickly becoming a frequent diagnosis for many older adults. It can be a precursor to a more serious cognitive diagnosis later and should not be ignored or dismissed. We created CONNECTIONS to help those living with MCI avail themselves of every opportunity to slow memory loss and forgetfulness. 

CONNECTIONS is for seniors who are exhibiting early signs of memory loss such as: 

  • Losing things often 
  • Forgetting important events 
  • Having trouble making decisions 
  • Becoming easily overwhelmed 
  • Having trouble finding the desired words 
  • Increased confusion

Personalized Care Plans 

Our CONNECTIONS Directors work with each resident to personalize their program, offering support where needed and independence where capable.  Thoughtful engagement designed to enrich life and develop a person’s interests are part of the daily routine and is customized depending on the interests of the group.  As early intervention can help slow further decline. Slowing Memory Loss is Possible

Neuroplasticity is the ability to form new connections in the brain.  This can help heal damage from an injury or illness and can also help slow memory loss. Like how physical workouts need to stay varied so your whole body gets well-toned, cognitive workouts need to adapt to improve pathways. You can strengthen these pathways to help slow memory loss by: 

  • Managing health conditions 
  • Getting good sleep 
  • Managing stress 
  • A healthy diet with healthy fats, fruits, vegetables and whole grains

Building Social and Cognitive Connections

When socially isolated our brain connectivity declines; there are physical changes in brain structure when we don’t stay socially connected.  Social connection also affects sleep patterns, mental health, and mortality.   

Many early brain programs, such as CONNECTIONS, combine social interaction with physical activity and cognitive engagement, building both physical and cognitive connections at the same time.  Along with improving memory, social connection is also demonstrated to reduce stress and feelings of isolation and improve depression.
